Die von Emil Kraepelin vor mehr als hundert Jahren formulierte und konzeptualisierte Dichotomie zwischen „Dementia praecox“ und „manisch-depressivem Irresein“ steht prototypisch für die psychiatrischen Klassifikationssysteme auch zu Beginn des 21. Jahrhunderts. Die zentrale Grundannahme der Kraepelinschen nosologischen Taxonomie postuliert, dass psychische Störungen nicht nur auf einer Achse — der psychopathologischen Phänomenologie — gruppiert werden können, sondern in einer festen Beziehung zu anderen Variablen wie Schweregrad, Verlauf oder Komorbidität mit anderen Erkrankungen stehen. Danach lassen sich psychische Störungen in distinkte Krankheitsentitäten einteilen, denen eine je eigene Ätiologie zugrunde liegt.
