Das chronische Nierenversagen ist definiert als irreversible, meist progrediente Einschränkung der Nierenfunktion unterschiedlicher Ätiologie bis hin zum dialysepflichtigen Terminalstadium. Diese Entwicklung wird begleitet von kompensatorischen Prozessen, die ihre eigene klinische Symptomatik haben. Meist ist die zur Niereninsuffizienz führende Grunderkrankung in der Phase der chronischen Niereninsuffizienz bekannt. Es handelt sich entweder um primäre Nierenerkrankungen (z. B. Glomerulonephritis, bakterielle oder nichtbakterielle interstitielle Nephritis, polyzystische Nierenerkrankung) oder sekundäre Nierenerkrankungen im Rahmen von Systemerkrankungen (z. B. Lupus erythematodes, M. Wegener, Amyloidose) bzw. Stoffwechselerkrankungen (z. B. Diabetes mellitus). Von Bedeutung ist das Ausmaß der morphologischen Veränderungen in beiden Nieren zum Zeitpunkt der Diagnosestellung im Hinblick auf die Behandelbarkeit und Prognose. Dabei sind für therapeutische Überlegungen die tubulointerstitiellen Veränderungen (z. B. das Ausmaß der interstitiellen Fibrose) mehr als die glomerulären Läsionen von Relevanz.
