Die Pathogenese autoimmuner Lebererkrankungen ist unklar, wird aber durch einen Toleranzverlust des Immunsystems gegenüber dem eigenen Lebergewebe verursacht. Ätiologisch werden hier virale und bakterielle Auslöser (Mimikry-Hypothese) impliziert, die vor dem Hintergrund einer polygenen immungenetischen Prädisposition des Betroffenen (HLA-Gene und nicht-HLA-Gene) zur Überwindung der immunologischen Toleranz führen. Schlüssige Beweise für diese Effekte liegen für die Autoimmunerkrankungen der Leber nicht vor.
