Grundlage der Spina bifida ist eine Fehlbildung des Neuralrohres. Diese führt zu einer angeborenen Querschnittssymptomatik mit unterschiedlichem Befallsmuster. Entscheidend ist die Höhe und die Ausprägung der Schädigung (komplette oder inkomplette Ausfallssymptomatik). Durch einen begleitenden Hydrocephalus kann die Koordination und intellektuelle Leistungsfähigkeit beeinträchtigt sein. Dies ist bei komplexen Versorgungen zu berücksichtigen. Sensibilitätsstörungen sind immer vorhanden.
