Gesundheit und soziale Lebenslage: Herausforderung für eine inklusive Gesundheitsversorgung

Zusammenfassung

Chronische soziale Benachteiligung bzw. Armut gehen bei Erwachsenen mit reduzierter Gesundheit und geringerer Bildung und bei betroffenen Kindern zusätzlich mit einer geringeren neurokognitiven Entwicklung einher (vgl. Case/Lubotsky/Paxson 2002). Der soziale Gradient für Gesundheit ist empirisch vielfach nachgewiesen worden. Es besteht wissenschaftlich Einigkeit darüber, dass dieser Zusammenhang über die gesamte Lebenspanne geht und alle demographischen Gruppen betrifft. Dieser Zusammenhang ist unabhängig davon, wie in den verschiedenen Untersuchungen Armut bzw. soziale Klasse definiert wurde (vgl. Feinstein 1993). Überwiegend wird in der Literatur von einer kausalen Beziehung ausgegangen (vgl. West 1991). Wissenschaftliche Untersuchungen beschäftigen sich vor allem mit Unterschieden in den sozioökonomischen Merkmalen wie Ausbildung, beruflicher Status und Einkommen, wobei gleichzeitig einhergehende Unterschiede im Gesundheitszustand auch als „gesundheitliche Ungleichheit“ bezeichnet werden (vgl. Mielck 2000). Neuere wissenschaftliche Ansätze differenzieren zusätzlich nach Geschlecht, Familienstatus, Ethnizität und Migration, sozialer Integration, regionalen Bezügen, Verstädterung und Teilhabe an kulturellen Aktivitäten. Zunehmend wird auch eine größere Bedeutung der subjektiven Bewertung der sozialen Situation durch die Akteure und Akteurinnen selbst eingeräumt. (vgl. Kolip/Koppelin 2002).
