Zusammenfassung
Die medizinische Fachterminologie wendet den Begriff Schmerz überwiegend auf körperliche Empfindungen an. Dagegen hat Schmerz in der deutschen Umgangssprache eine sehr viel weitere Bedeutung, die von Trauer bis zu eng lokalisierten, körperlichen Mißempfindungen reicht. Noch ursprünglicher und umfassender verwenden einige fremde Ethnien ihren Begriff für Schmerz. Dies soll im folgenden am Beispiel der Cabuntogueños, Einwohner einer Fischergemeinde im Süden der Philippinen, dargestellt werden. Sie verstehen unter masakit nicht allein physische Schmerzen, sondern sprechen davon, daß chronische Krankheiten wie Geistesschwäche oder Blindheit ‘schmerzen’, die nach unserem Verständnis nicht von Schmerzen begleitet sind. Ein Rückblick in die Geschichte zeigt, daß früher auch im abendländischen Kulturkreis ein ähnlicher weiter Schmerzbegriff vorgeherrscht hat.
Da die Hilflosigkeit gegenüber bestimmten Krankheiten schmerzverstärkend wirkt, können Mittel, die die Hilflosigkeit reduzieren und die Kontrollüberzeugung stärken, schmerzlindernd wirken. Sie fördern das allgemeine Wohlbefinden. Dabei ist es belanglos, ob wirksubstanzgebundene oder magische Methoden eingesetzt werden. Dies ist ein Grund, weswegen magische Therapien bei den Cabuntogueños immer noch mit Erfolg angewandt werden.
Summary
Respondents and methods: In a field study lasting more than 8 months aspects of the perception of illness in a fishing Community in the northeast of Mindanao (Philippines) were investigated. 100 individuals served as respondents. From frequently occurring illnesses known to the fishing men 54 concepts were selected. The respondents were asked to order the illness concepts according to certain criteria (for instance according to the intensity of pain, fear, helplessness facing an illness or according to the availability of means of treatment, costs of treatment, infectiousness, dangerousness, prognosis, etc.). In this way the Cabuntoguenos’ judgement of 24 aspects, i.e. attributes or qualities of illnesses, were collected. Computer-aided regression analyses were employed to analyze the more than 60.000 rank-ordered data points in an effort to find mutual interdependence between the attributes of the perception of illness. Furthermore, multidimensional scaling was applied to the data and the criteria identified that underly the ordering and Classification of the illness categories.
Results: In Cabuntog pain (masakit) includes more than physical pain alone. Especially chronic diseases, as mental deficiency of blindness hurt. The concept of pain of diseases has a much wider meaning than in our own society. This is not to say that people complain more frequently of pain while presenting their complaints to healers. Cabuntogueños rarely express pain. Illnesses with high costs of treatment are seen as especially painful. The pain of illnesses correlats strongly with the knowledge that one would be helpless due to insufficient financial means. The knowlegde that medical and technical possibilities of treatment exist but carmot be accessed due to poverty is pain-increasing. Painful illnesses are seen as dangerous and tend to have a bad prognosis; reasons why they are feared. Helplessness in the face of certain diseases can be seen as pain-increasing. Methods with both employ effective substances and magical means and which strengthen one’s feeling of control and reduce one’s helplessness have the effect of lowering pain.
