Zusammenfassung

Dementielle Syndrome können sehr verschiedene strukturelle Korrelate haben. Man lernt in den letzten Jahren immer besser, die Form und den Verlaufstyp von dementieilen Erkrankungen mit den strukturellen Veränderungen zu korrelieren, die man im Gehirn vorfindet. Diese könnten entweder in vivo mit den bildgebenden Techniken dargestellt oder postmortal morphologisch nachgewiesen werden. Kennzeichnend ist die fluktuierende Symptomatik bei den vaskulären Enzephalopathien bzw. Demenzen, dagegen die kontinuierlich zunehmende, progrediente Symptomatik sowie der psychische Verfall bei der senilen Demenz vom Alzheimer-Typ. Diesen beiden Verlaufsformen hegen ganz unterschiedliche Hirnveränderungen zugrunde. Die klinisch-pathologische Korrelation läßt sich immer deutlicher herstellen, doch gibt es auch dabei noch sehr viele ungeklärte Fragen.
