Abstract

Primary mesenchymal tumors arising in the mediastinal compartment are rare. The occurrence of such tumors is by no means limited to a specific compartment, as these tumors have been described in the anterior and posterior mediastinum. In addition, in terms of the variety of these tumors in the mediastinal compartment, the gamut of soft tissue neoplasms in the mediastinum is similar to that observed in the soft tissues, with some minor differences that are intrinsic to this particular anatomic location. In this chapter, we have divided tumors based on the type of tissue present or the specific differentiation of the neoplasm, as far as our current knowledge permits, and have provided an analysis of these tumors in the mediastinum. This chapter will highlight the following neoplasms: adipose tissue tumors; vascular tumors; smooth and skeletal muscle tumors; fibroblastic, myofibroblastic, and fibrohistiocytic tumors; biphasic (epithelial/mesenchymal) tumors; chondroid and osseous tumors; and other mesenchymal neoplasms. This chapter will be presented from the practical point of view for the practicing pathologist who may be called upon to interpret these tumors. Clinical, pathological, histopathological, immunohistochemical, radiological, gross, molecular features will be discussed, as well as differential diagnoses and staging.
