Abstract

Patellofemoral arthritis is clinically challenging due to the complex and unique morphology of the PF joint, the heterogeneity of the articular surface among patients, and the high shear stresses across the joint. The pathogenesis of PF arthritis can generally be attributed to mechanical causes that overload the cartilage and include trauma, patellar instability, malalignment, and dysplasia. Chondral lesions of the patellofemoral compartment are common, and patients with symptomatic cartilage lesions of the patella and/or trochlea refractory to nonoperative treatment may be candidates for surgery. Recent advancements in cartilage restoration techniques and PF arthroplasty implants have resulted in improved clinical outcomes, thus expanding the indications of surgical treatment for patients with PF chondral defects and osteoarthritis.
