Abstract
The care and support for older people is so complex that multidisciplinary working is inevitable, not only within certain organizational settings but also between professionals from different settings and with informal caregivers. The role of informal caregivers is important, and professionals need to prevent them from caregiver burden, for themselves and for the older person they take care for. Taking care and support older people means taking care for their family members as well. A system approach is needed. An older person has presumably an older partner as well. Physical problems like immobility of the partner may cause mental and social problems of the older person but may also affect the partner; they are dependent on each other. It is a coherent whole and professionals are experts in share issues. They need to collaborate with each other and with the older person with his family to offer integrated care. This chapter pays attention to collaboration of health and social professionals mutually but also to collaboration with older persons and their informal caregivers.
