Abstract
This chapter is dedicated to the cooperation of independent health care providers. They combine knowledge and skills to achieve solutions for all aspects of care for people with DFS, knowledge and skills each member alone would not be able to achieve. In this chapter we’ve combined the experience of the authors in their own environments and in the networks they support. The result should be considered a resource of ideas and possibly even some solutions.
