Should Varicoceles Be Managed Surgically or Radiographically? (Surgery)

Abstract

Varicocele treatment has existed for over a century, and numerous surgical and radiographic treatment options exist. All available treatments provide reasonable improvements in clinical outcomes for the infertile male, although certain treatment choices may be preferred in unique patient populations or specific clinical situations. However, a clear difference exists among treatment methods regarding complication and recurrence rates, with microsurgical inguinal or subinguinal varicocelectomy using intraoperative Doppler assistance providing superior outcomes. Yet, recurrent varicoceles may be preferentially treated with a radiographic approach in certain situations. In the end, surgical treatment may provide a more overall cost-effective approach than radiographic embolization in the treatment of varicocele-related male infertility.
