Abstract

You are evaluating a 3-year-old boy in the ED for his lethargy. His family reported that he has been irritable, sleepy most of the day and he cannot get up and walk one day before his admission to the ER. His past medical history is unremarkable and his development is on target. On examination, he is lethargic, and difficult to arouse. He has drooling, intact gag, some inward deviation of his right eye, and no papilledema. He moves all four extremities equally, but overall has a paucity of movement. DTRs are decreased, but toes are up-going bilaterally. An urgent CT head is ordered, followed by a CT Venogram.
