Abstract

In the majority of adult congenital heart disease (ACHD) conditions, the patterns of heart failure (HF) closely mirror those seen in acquired disease. However certain conditions present unique physiology—these include those with a right ventricle (RV) in the sub-aortic position (systemic RV). As patients with systemic RVs age, their risk of heart failure significantly increases. There are multiple possible mechanisms that drive ventricular dysfunction, effort intolerance and overt heart failure. Some of these are very specific to the transposition patient such as baffle function and sub-aortic RV physiology. This chapter describes the factors that may contribute to the development of HF and the possible therapeutic options.
