Laparoscopic-Vaginal Radical Hysterectomy

Abstract

Vaginal surgery and laparoscopic surgery are the two minimally invasive approaches available to the gynecologist. The former has been pioneered in the nineteenth century; the latter has been developed during the last decade of the twentieth century. The laparoscopic-vaginal radical hysterectomy has been in a recent past the “missing link ” between vaginal and laparoscopic surgery. Designed by surgeons trained in both innovative laparoscopic and classical but confidential vaginal radical hysterectomy techniques, it allowed to revive the elegant vaginal surgery, which has the advantage of exactly tailoring the vaginal excision, and at the same time to overcome its shortcomings, which were the need for an episiotomy and the impossibility to perform a lymph node dissection. While the laparoscopic-vaginal radical hysterectomy has been largely replaced by the full laparoscopic radical hysterectomy, some technical details may be useful to know in order to solve specific surgical problems in particular patients, e.g., the patient who underwent a simple hysterectomy for invasive cervical cancer and needs a colpectomy and parametrectomy. In addition, there is a need to maintain the skills for the radical vaginal surgery necessary to complete a radical vaginal trachelectomy, which remains the gold standard for fertility-preserving surgery.
