Abstract
The increasing bulk of chronic diseases requires that patients are continuously monitored, even when they are discharged from the hospital and go home to continue treatments. However, there is still poor integration between hospital patient records and systems supporting patients’ care at home that are mainly based on mobile applications. High-quality home treatment requires an integrated management of care, involving both the electronic health record (EHR) of the hospital and the mobile applications on the patient/family side.
