Abstract
Anterior mediastinotomy gives access to abnormalities of the mediastinum lying in front of the great vessels and also to those that lie below and lateral to the carina. In particular the subaortic fossa, the site of lymphatic spread from left upper lobe tumours, can be examined. These are all areas that cannot be approached (or can only be approached with great difficulty) via a cervical mediastinoscopy. This procedure has an additional advantage in that the pleura can easily be opened and biopsies can be obtained from the hilar region of the lung. An alternative is VATS.
