Abstract
The pleural cavity is entered through a left posterolateral thoracotomy, stripping the periosteum from the lower border of the fifth rib. Adhesions between the lung and the parietal pleura are divided and the apex of the lung is retracted downwards and laterally. The mediastinal pleura is incised behind the phrenic nerve and over the pulmonary artery, which is the highest structure in the hilum of the lung.
