Abstract

Pectoralis major transfer is a valuable salvage procedure for isolated, irreparable subscapularis tears in young patients. In other conditions like in the revision of isolated subscapularis failures after anatomic shoulder replacement, or if a massive anterosuperior rotator cuff tear is associated with pseudoparalysis of anterior elevation, it appears that a reverse shoulder arthroplasty provides better clinical results.

This case highlights the steps of the pectoralis major transfer for irreparable subscapularis tears. The anatomy, indications, biomechanics, different surgical techniques, and outcomes are also reviewed.
