Abstract
Occipital surgical approaches may be already performed be young neurosurgeons in training. Hence, this region is presented with great detail.

Orientation using X-ray and MRI can be supplemented by surgical landmarks. Medial structures such as the cuneus, precuneus, and the sutures are useful for orientation. Exact lateral landmarks include only the upper point of lambdoid suture and the parietooccipital sulcus.

Besides angiographic details, spatial anatomical drawings of the relevant occipital structures are presented.

Selected findings using MRI and neuronavigation are demonstrated.
