Abstract
Laparoscopic hernia surgery has certainly evolved since its initial description in the early 1980s. Today it is now accepted as a good option to the traditional open repair. There are many benefits associated with the minimally invasive approach, but these benefits are countered with the increased learning curve and expense associated with the laparoscopic approach. The two described laparoscopic approaches described here in this chapter are the totally extraperitoneal and the transabdominal preperitoneal approaches. Both have been argued to be the better surgical approach. However, most data trends reveal that both are good options. The best method will be based upon individual surgeon preference and the outcomes achieved. Whichever approach is chosen, the surgeon must have a very good understanding of the inguinal anatomy to have success.
