Bonding to Tooth Tissues

Abstract

Apart from self-adhesive (self-adhering) composites, all dental composites bond to tooth tissues via adhesive systems. In this chapter, characteristics of the adhesive-tooth and adhesive-composite bond are discussed with an emphasis on the clinical factors affecting the adhesive-dentin bond as the most challenging in contemporary adhesive dentistry. It is now widely accepted that the weakest link of the composite-tooth restoration remains the hybrid layer. Understanding of the resin-dentin bond degradation mechanisms and strategies for improvement are important determinants of the clinical longevity of composite restorations. Two main mechanisms of resin-dentin bond degradation, hydrolysis of the resin matrix and enzyme-mediated collagen degradation are explained. Strategies for improved resin-dentin bond include inhibition of enzyme-mediated collagen degradation, cross-linking agents for collagen strengthening, bonding techniques for improved polymerization as well as therapeutic remineralization of resin-dentin interfaces.
