Abstract
Trimalleolar ankle fractures with a posterior malleolus component are typically the result of a low-energy mechanism of injury. There is continued debate on indications for fixing the posterior malleolus and optimal fixation construct for the posterior malleolar fragment. Some studies have suggested that fixation of this fragment restores articular congruity and joint reactive forces and optimizes reduction of the syndesmosis. This chapter reviews considerations in treating posterior malleolar fractures, operative technique and technical pearls, and postoperative care.
