Abstract
Bunion surgery is a common procedure performed by most foot and ankle surgeons. Despite decades of published clinical research, there remains extremely high variability in the evaluation and management of the deformity with hundreds of procedural solutions. Lack of consensus exists for nearly every aspect of the topic including etiology, terminology, procedure selection, radiographic evaluation, and interpretation of surgical outcomes. In this chapter we discuss many of the shortcomings of medical literature that introduce bias into our understanding of treatment outcomes and highlight the need for more stringent application of evidence-based principles for management. This overview will serve to set the perspective for the following chapters examining the current evidence base on bunion surgery. Only through recognition and understanding of the shortcomings of our perspectives can we arrive at a true answer to solve this problem for our patients.
