Abstract

Marijuana is the most widely abused illicit drug among pregnant women. A large majority of women who abuse marijuana also report abusing other licit and illicit drugs. The respiratory effects in the long-term user are similar to those of long-term tobacco abuse. Because polysubstance abuse is common, the effect of cannabis on obstetric and neonatal outcomes is difficult to determine; however, little evidence for an increased risk for congenital anomalies or preterm birth exists. Obstetric management is little altered. Anesthetic management is similar to that in persons who abuse tobacco, and post-cesarean pain management may be more difficult.
