Reduction Mammaplasty with a Circular Folded Pedicle Technique

Abstract

The evolution in the last 12 years of a personal technique for reduction mammaplasty is presented. First, the periareolar pedicle is marked, and then the inferior pole, where three types of marking are possible: type 1—linear marking; type 2—vertical or oblique fusiform marking; type 3—transverse fusiform marking, which with the periareolar marking is similar to a cyclops figure. The technical sequence is as follows: the handling of the pedicle, the reverse flap procedure, and then the breast resection.

The most important advancement was the development of a reverse flap in the inferior pole to improve a high and hypotrophic submammary fold. This flap has a transverse fusiform shape corresponding to the mouth of a Cyclops. After an incision in the superior border, the flap is rotated inferiorly, pushing down the flap tissue to make the full submammary fold. Nowadays, this flap procedure is performed in most patients with hypertrophy and ptosis.
