Abstract

Pseudofolliculitis barbae (PB), acne keloidalis nuchae (AKN), dissecting cellulitis (DC), and hidradenitis suppurativa (HS) are chronic inflammatory disorders of hair-bearing areas that commonly affect individuals of African ancestry. They can be differentiated based on location (beard area versus scalp) and the presence of keloidal plaques, abscesses, and draining sinus tracts. HS is a disease that affects apocrine-gland bearing areas and predominately presents as comedones, nodules, abscesses, and draining sinus tracts in the axilla, groin, and buttocks. Although the underlying pathogenesis differs, the primary etiology of these conditions appears to involve occlusion and rupture of the hair follicle, resulting in an inflammatory response. Therapy is directed at preventing follicular occlusion, treating and preventing secondary infection, and interrupting the chronic inflammatory response that perpetuates these disorders. In extreme cases, surgery may be required. Understanding the epidemiology, pathogenesis, clinical presentation, and treatment of these conditions is essential for directing patient management and will be addressed in this chapter.
