Cord Blood Transplants Versus Other Sources of Allografts: Comparison of Data in Adult Setting

Abstract

In the absence of a human leukocyte antigen (HLA)-matched related donor, umbilical cord blood (UCB) provides an excellent alternative graft source for hematopoietic cell transplantation. It is, however, associated with delayed neutrophil engraftment leading to a higher risk of graft failure and non-relapse mortality. On the other hand, despite HLA mismatch, the risk of acute or chronic graft-versus-host disease (GvHD) is significantly lower after UCB transplantation as compared with any other graft source with similar degree of mismatch. Moreover, the risk of relapse, disease-free survival, and overall survival after UCB transplantation is comparable to that of other graft types. Outcomes of UCB transplantation as compared with partially HLA-matched related donor (haploidentical) transplantation are currently being evaluated in a randomized clinical trial.
