Abstract
Transanal endoscopic microsurgery (TEM) represents the embodiment of natural orifice translumenal endoscopic surgery (NOTES). Its indications range from excision of benign lesions of the rectum to select cancers after neoadjuvant therapy. It has the advantage of resecting the lesion without entering the abdominal cavity which translates into less morbidity and mortality. TEM also offers chances for sphincter preservation in very low rectal lesions. We describe herein the indications of TEM and the workup of patients presenting with rectal tumors, and then, we explain the technical aspects of this procedure.
