Abstract

Papillary carcinoma shows a striking preference for the fingers and toes.

Histopathologically, the neoplasm is usually multinodular and poorly circumscribed, with no connection with the epidermis. Papillary carcinoma, despite this appellation, is a predominantly solid neoplasm, with only focally tubular structures with papillary projections.

Immunohistochemical studies have shown positivity for the S-100 protein in the cytoplasm of neoplastic cells, as well as the CEA and EMA along the luminal cells of papillae and tubular structures. Smooth muscle actin, p63, and calponin highlight a myoepithelial layer around tubular and glandular structures.

Papillary carcinoma is an aggressive neoplasm, and excision should be performed with clean margins. Several cases of papillary carcinoma described in the literature developed distant metastasis.
