Abstract
Parathyroid glands have a predominant role in mineral homeostasis. Calcium and vitamin D are two of the main regulators of the parathyroid function by virtue of the presence of the calcium-sensing receptor and vitamin D receptor in parathyroid cells. FGF23 also modulates parathyroid function. Chronic renal failure is characterized by derangements in factors involved in mineral metabolism that lead to the development of secondary hyperparathyroidism. This chapter reviews in depth the pathophysiology of secondary hyperparathyroidism, the function and regulation of the vitamin D receptor and the calcium-sensing receptor, as well as their roles in the development of parathyroid hyperplasia.
