ACL and Posterolateral Instability

A consensus on the treatment of combined anterior cruciate ligament (ACL) and posterolateral corner (PLC) injuries is still lacking. The purpose of this chapter is to discuss the current state of combined anterior and posterolateral instability, which is arranged as follows: (1) pathophysiology and biomechanics to provide a framework for further discussion; (2) clinical evaluations including the key features of the history, physical examinations, imaging studies, and diagnostic arthroscopy that will enable the surgeon to appropriately evaluate these injuries and successfully move forward with management; (3) current available surgical strategies for combined ACL/PLC injuries; and (4) a suggested treatment algorithm and approaches for both the acutely and the chronically injured patient.
