Abstract

Tissue hypoxia as a result of arterial insufficiency can lead to tissue injury and loss. Compromised tissue due to ischemia, even after prompt restoration of arterial flow, following acute injury can be further damaged through the complex processes of reperfusion injury. Threatened tissue due to chronic limb ischemia can manifest as chronic ischemic wounds and rest pain. Hyperbaric oxygen therapy (HBO) is an important tool in hypoxic tissue management by significantly increasing plasma oxygen concentration and oxygen capillary diffusion distance. This chapter will review important physiologic mechanisms of HBO in the management of ischemic injury as well as potential applications to clinical scenarios of ischemia, including critical limb ischemia.
