Abstract
Important research topics considering young people’s transition support needs include the persons’ attitude towards transition and their experiences; the timing of transitions and of needed support and influencing factors; determinants of positive transition experiences and the relation with health, healthcare and psychosocial outcomes; outcomes for successful transition to adult care and adulthood; the development, use and evaluation of transition interventions; self-management; and the development of quality of life over time. The increasing attention for transition issues in young people with congenital heart disease (CHD) and their being recognized as a distinct group with specific needs is applauded. However, the overall message for the adolescent CHD health field is that there is still a lot to do and to achieve. The time has come for action.
