Abstract

Successful transition from pediatric care to adult care depends on providing specific information for patients, parents, and attending physicians at an early stage, professional endorsement of the patient during the transition process, and transfer of comprehensive medical records to the adult specialist. We describe several toolkits which have been shown to be conducive to a well-regulated transition.
