Incarcerated Hernias

Abstract

Incarceration is the state of an external hernia which cannot be reduced into the abdomen. It implies an increased risk of small bowel obstruction and should be considered a surgical emergency. In literature, some studies support the role of laparoscopy in emergency groin hernia repair. A Cochrane library study performed in 2003, in fact, showed that the outcome of elective laparoscopic GHR is at least equivalent to that of the open approach. Concerning the different laparoscopic approaches to the emergency groin hernia repair, apart from the classical TAPP and TEP methods alone, many variants have been described, which are known as “combined” or “hybrid” treatment, where laparoscopy may have many possible roles, from the diagnostic evaluation of the bowel viability to a time of the surgical manipulation. We will try to clarify, in this chapter, the peculiar aspects of the laparoscopic approach to the emergency groin hernia repair.
