Abstract
In this chapter, the author addresses the role of assisted living facilities in a geriatrics practice. While giving a historical perspective on reimbursement in the care of assisted living residents, the author describes the improved reimbursement opportunities that exist today. He also discusses the dichotomy of the medical and nonmedical model that has fluctuated in assisted living facilities. The importance of placing a patient in the most appropriate level of care is discussed in the context of housing choices. Opportunities to effectively interact and improve coordination of care the role of wellness nurses and how they relate to the practice are reviewed. Finally, the concept of creating a bridge between the patient family and staff is discussed in the context of effective chronic care management.
