Abstract
A 25-year-old patient had bilateral breast augmentation. She had a moderately wide intermammary distance and thin upper pole (pinch test). Six months postoperatively she was noted to have descent that increased the following year. The discussion centered around a variety of procedures including capsulectomy or capsulotomy, closure of the inframammary fold, and the size of the implant. Revision was performed with good results.
