Abstract
Total ankle replacement is a technically challenging reconstruction, with soft-tissue complications posing potential significant morbidity, especially when the anterior ankle soft-tissue envelope is hostile secondary to scarring and a suboptimal soft-tissue envelope for healing. Alternate approaches to the ankle for arthroplasty may need to be sought in unique cases. The authors describe a modified approach to the anterior ankle, lateral approach, as well as posterior surgical approach for complex primary and revision total ankle replacement.
