Overview of Outcomes with Accelerated Partial Breast Irradiation

Abstract

Historically, randomized trials and meta-analyses have demonstrated a benefit in local control, and breast cancer mortality with adjuvant whole breast irradiation (WBI) following breast-conserving surgery (BCS). However, review of patterns of failure in patients undergoing breast-conserving therapy has demonstrated that the majority of local recurrences occur within close proximity to the lumpectomy cavity. Accelerated partial breast irradiation (APBI), which is based on the concept of radiotherapy to the area surrounding the lumpectomy cavity, has emerged as a treatment option for women with early-stage breast cancer over the past several decades. By limiting the volume of normal tissue treated, APBI has the potential for reduced toxicity, but also improved patient convenience as treatment is completed in 1 week or less. This directly addresses the underutilization of breast-conserving therapy and offers a solution to the large number of patients who fail to complete the 5–6-week course of adjuvant external beam WBI due to social and economic considerations.
