Abstract

True and false diverticula can occur at any portion of the esophagus. Zenker’s pharyngeal and epiphrenic distal diverticula are by far the most common. There are both surgical and endoscopic approaches to both, with differing indications. Most Zenker’s are today treated using a NOTES approach. There is growing interest in flexible endoscopic transoral diverticulectomy as opposed to the more common ridged transoral approach as it is applicable to all patients and involves no specialized equipment. The role of endoscopy in other esophageal diverticula is mostly as an adjunct to laparoscopic/thoracoscopic approaches—but this may change as endoscopic technology evolves.
