Surgery for Parkinson's Disease

Abstract

Potential surgical candidates with Parkinson’s disease (PD) are first preselected by a movement disorders neurologist, subsequently seen by multiple specialists, and ultimately rendered as surgical candidates through a series of discussions by each member of the multidisciplinary team during dedicated patient management conferences. Patients considered for a full presurgical evaluation are those with a clear diagnosis of progressive idiopathic PD and an adequate response to levodopa. The current state of surgery for movement disorders, particularly DBS, is in constant change. New technologies, in particular, neuronavigation, frames, and leads, fill up the neurosurgeon’s armamentarium with several options when providing care for these patients. This chapter provides a description of the indications, preoperative evaluation, targeting, and an overview of the technical nuances of DBS for PD, along with its complications.
