Abstract
Chronic anal pain represents a diagnostic and a therapeutic challenge for the practitioner. Frequently neglected, routinely understudied, poorly understood, and poorly treated, the condition results in considerable morbidity for the patient. This article provides a stepwise diagnostic algorithm to identify the etiology of the anal pain and—as much as feasible—data-driven recommendations for treatment, with special focus on functional etiologies such as levator ani syndrome, myofascial pain, and proctalgia fugax.
