Abstract

Having punctured the kidney and established a guide-wire, the surgeon now has to create an operating conduit, find, break and remove calculi and establish postsurgical drainage.Although techniques and instruments vary, the operative principles remain the same, in particular the need for patience, avoidance of haste, gentle instrumentation and alternative plans when the situation changes, as it often does.This chapter describes tract dilatation, nephroscopy, stone fragmentation and extraction. This includes the technique for the establishment of a universal guide-wire and nephrostomy insertion.
