Is NIV Safe and Effective in Patients with ACPE Due to Diastolic Dysfunction?

Abstract

No previous studies have clarified the role of noninvasive ventilation (NIV) in patients with acute cardiogenic pulmonary edema (ACPE) and preserved systolic function. After a cardiac ultrasound evaluation to exclude ACPE patients with depressed systolic function and valvular abnormalities, 20 “diastolic ACPE patients” were randomly assigned to receive NIV plus 1.8 mg of nitrate (Group A, n = 10) or NIV plus 4.8 mg of nitrate (Group B, n = 10). Five patients from Group B (50 %) showed a transient arterial hypotension compared with one patient from Group A (p < 0.05), but no significant difference was seen in the two groups with regard to in-hospital mortality or discontinuation of NIV. One patient in group A required endotracheal intubation. This preliminary study showed that the application of NIV in patients with ACPE due to diastolic dysfunction seems to be safe and effective regardless of nitrate dosage.
