Abstract

The anterior approach to hernia repair without mesh is typically applied to patients with contraindications to mesh implantation, patients who do not wish to have mesh implanted, or is used among surgeons and in centers that preferentially provide the non-mesh hernia repair. The Shouldice, Bassini, and McVay procedures are the most commonly performed. Complications after this repair can be due to hernia recurrence, direct nerve entrapment or injury, or abdominal wall stiffness. Chronic pain is a common factor after tissue repair; however, unlike mesh repairs, the incidence dramatically reduces with time, and supportive therapy alone may suffice.
