Abstract
During my undergraduate training, I had no difficulty in recording the sections of a patient’s history that required responses to specific questions, such as demographic data, family history, and review of systems. However, I did have problems in recording the sections of a patient’s history that required a narrative report, namely, the patient’s personal/psychosocial history and history of the present illness.
