Abstract

The lesbian, gay, bisexual, transgender, and intersex (LGBTI) population has struggled against a history of discrimination and marginalization, which includes the healthcare system. In an attempt to bridge this gap, healthcare providers should strive to provide an inclusive and welcome clinic environment for members of this community. Success will take substantial thought and effort by healthcare providers and organizations, and the reward will be a strong therapeutic relationship between a provider and the LGBTI patient. The clinical environment can be broken down into three broad categories: physical, systemic, and interpersonal. Addressing the physical environment includes providing LGBTI friendly décor and materials to create a welcome space. Practicing under a comprehensive mission statement, establishing inclusive non-discrimination and visitation policies, and creating a comprehensive intake form for a diverse population will guide operations within the systemic environment. Finally, ensuring that staff is trained to understand and respond to LGBTI issues, and creating a system to address and overcome obstacles to equitable care are key components of the interpersonal environment. Once these changes have been implemented, advertising and community outreach can further help encourage LGBTI individuals to engage with the healthcare system. The goal should always be equality in health and wellness, and improving access for a historically underserved population.
