Abstract

A functioning adrenal axis is a prerequisite for survival in critical illness. In this chapter we describe basic adrenal physiology, and the presentation and treatment of adrenal crisis. Additionally we discuss some more controversial aspects of adrenal function in critical illness: the concept of “critical illness-related corticosteroid insufficiency”, and the role of plasma cortisol measurements and the short Synacthen test in intensive care, as well as the place of hydrocortisone treatment in septic shock.
