Abstract
A larger-than-normal eye faces several risks ranging from RD to CNV. The VR surgeon has some influence on some of these risks, but the choice of the proper intervention requires careful decision-making. If cataract develops, the standard “phaco + IOL in the bag” has valid alternatives, and prophylactic (endo) laser cerclage should always be considered. Vitrectomy may require modified techniques in these eyes, especially if an RD over a posterior staphyloma is present.
